Agreement

1. I understand that I fails to abide by rules and/or commits misconduct, i will be sent home at my expense and I agree to pay UPM all such travel costs.

2. I understand that if i causes any damage to UPM property or another person property, then I will be charged accordingly.

3. I consent to any emergency treatment for my child deemed necessary in case of accident or illness.

4. I agree to abide by the conditions of enrolment. I am responsible for the payment of all fees for the

 abovenamed participant.

5. I agree and consent to the participation of the student in all supervised activities.

6. I understand that my photograph during camp activity may be used for educational or

 promotional purposes. Video recordings may also be used for educational or promotional purposes.

7. I have read and accept the terms and conditions and agree to abide by them.
